Three workers dismissed by Metro Vancouver after fatal incident at Cleveland Dam

VANCOUVER – Three employees have been dismissed by Metro Vancouver after the fatal release of water from the Cleveland Dam earlier this month.
The regional district says in a news release that the incident happened during maintenance work on a drum gate and was due to human error.
Metro Vancouver says it is co-operating with agencies investigating what happened.
Preliminary findings previously released by Metro Vancouver say the error was related to the programming of the control system for the spillway gate at the dam.
Five people who were fishing downstream were swept away when a large volume of water rushed from the dam down the Capilano River on Oct. 1.
The body of one man was pulled from the river and a second man hasn’t been found.
